Scaling and Root Planing: A Deep Cleaning for Healthier Gums

Scaling and root planing — sometimes called a “deep cleaning” — is a non-surgical treatment for gum disease (periodontitis) that goes beyond a routine dental cleaning. A standard cleaning handles plaque and tartar above the gumline, but when gum disease is present, harmful bacteria have worked their way below the gums and onto the surfaces of the tooth roots. That’s where scaling and root planing comes in. Using specialized hand instruments and ultrasonic tools, your dental provider carefully removes built-up plaque and hardened tartar (also called calculus) from beneath the gumline. Then comes the planing step, in which the root surfaces are gently smoothed. A clean, smooth root gives your gums the chance to heal and reattach snugly to the tooth, closing off the deep pockets where bacteria thrive.

Why Scaling and Root Planing Is Recommended

Gum disease progresses quietly. It often begins as gingivitis — red, puffy gums that may bleed when you brush — and if left untreated, it can advance into periodontitis, where the infection damages the bone and connective tissue that hold your teeth in place. At that stage, the gums start pulling away from the teeth, forming pockets that trap even more bacteria, and over time the result can be shifting teeth, chronic bad breath, and eventually tooth loss. The American Dental Association recommends scaling and root planing as the first-line, non-surgical treatment for chronic periodontitis, and peer-reviewed systematic reviews have consistently shown it provides a moderate but meaningful benefit — reducing pocket depth, decreasing inflammation, and helping gums reattach to cleaner root surfaces. Treating gum disease early also matters beyond your mouth: research has linked periodontitis to systemic conditions like diabetes, heart disease, and adverse pregnancy outcomes, making timely care an investment in your overall health.

What the Procedure Is Like

Because the work takes place below the gumline, most patients receive local anesthesia to keep the area comfortable throughout the appointment. Depending on the extent of disease, the treatment is often split over two visits — typically one side of the mouth per appointment — so you’re never numb on both sides at once and your clinician has enough time to work thoroughly. You may feel some pressure or vibration from the ultrasonic scaler, but discomfort is usually minimal. Afterward, it’s normal to experience some tenderness, mild gum soreness, and temporary tooth sensitivity to hot and cold for a few days, and your gums may bleed slightly when you brush at first. Your dentist may also recommend an antimicrobial mouth rinse, a locally applied antibiotic placed directly into deeper pockets, or in some cases a short course of systemic medication to support healing. Sticking with gentle but consistent brushing, flossing, and any prescribed rinses during recovery makes a real difference in how well the tissue heals.

After Treatment: Ongoing Care Matters

Scaling and root planing gives your gums a fresh start, but it isn’t a one-and-done fix — gum disease is a chronic condition that needs ongoing management. About four to six weeks after the procedure, your dentist will reassess your gums to see how well they’ve responded, measuring pocket depths and checking for continued inflammation. If deep pockets remain or disease is still active, additional treatment may be recommended. Most patients who have had periodontitis are placed on a periodontal maintenance schedule — cleanings every three to four months rather than the standard six — because research shows this shorter interval helps keep bacteria from rebuilding in hard-to-reach areas. Daily home care is the other half of the equation: brushing twice a day, flossing or using interdental brushes, and avoiding tobacco (a major risk factor for gum disease) all help preserve the results of your treatment. With consistent care, many patients are able to stop the progression of gum disease entirely and keep their natural teeth for a lifetime.
